ABSTRACT

Regulation of the Minister of Health Number 37 of 2014 concerning Determination of Death and Utilization of Donor Organs Regulates the withdrawal or withholding of life support therapy. Even though there are rules, there are still problems in its implementation. The implementation shows the existence of the ethical dilemma of its implementation and resistance from the patient's family. This study was to see medicolegal implementation of the withdrawal or withholding of life support therapy in patients with terminal states treated at Ahmad Ripin Regional Hospital, the obstacles encountered, efforts made to overcome problems. Research uses sociological juridical research methods. Primary data obtained from interviews. Secondary data obtained from document studies. Data analysis techniques using qualitative analysis. The results of the study concluded: 1. Ahmad Ripin Regional Hospital does not yet have a special written protocol regarding the termination of overall withdrawal or withholding of life support therapy in the form of director regulations, 2. The main obstacle includes rejection from the patient's family and difficulty understanding the medical conditions of complex patients. From the internal side of the hospital, obstacles include the absence of director's policies, lack of ICU-certified medical personnel, as well as limited equipment, medicines, and the absence of palliative teams, 3. Efforts that have been made by doctors to overcome obstacles are intensive education and communication with patients and families and medical socialization of termination/delay of life assistance to related officers to the relevant officers
